Over 450,000 Insulin Syringes Recalled Due to Deadly Labeling Error

The Food & Drug Administration has issued a Class 1 recall of 471,000 Covidien ReliOn insulin syringes sold at Wal-Mart & Sam Club’s between July and October 2008. The hypodermic syringes have been mislabeled and as a result a patient could receive over double the dose of prescribed insulin. Class 1 recalls are reserved for situations in which continued use of the product could cause serious injury or death.

The syringes affected by the recall are sold under the brand name of ReliOn and are of the 1cc, 31-gauge variety. They are sold in packages of 100 units for use with U-100 insulin. Syringes labeled for use with U-40 unit insulin were placed in packages with syringes labeled for use with U-100 insulin. If a patient was to use a mislabeled syringe they could receive as much as 2.5 times the prescribed dose. The overdose of insulin could lead to hypoglycemia, abnormally low level of blood sugar, which could lead to serious health consequences or death.

If you believe you have been purchased the syringes in this recall return them immediately to your local Wal-Mart or Sam’s Club for a replacement product. Only those sold in Lot Number 813900 with a Product Code of 38396-0403-02 are being recalled.

BridgeStone/FireStone is Recalling Almost 165,000 Tires With Insufficient Tread Base Gauge

BridgeStone/FireStone is recalling 162,251 tires because the tires were manufactured with insufficient tread base gauge. Prolonged use of the tires can lead to tread vibration and groove cracking which will ultimately cause the tread to distort or separate. Tread detachment is a serious concern for drivers because it can lead to loss of vehicle control which ultimately ends in a serious auto accident. BFNT will notify owners of the recall replace, remount and balance new tires on consumer’s cars free of charge.

The tires in question are FIRESTONE/FR380/P235/75R15 manufactured between June 2006 and September 2007 and LEMANS/Champion SE/P235/75R15 manufactured between June 2006 and September 2006.

Nearly 200,000 Soccer Nets Recalled After Strangulation Death of a Child

Regent Sports is recalling 190,000 soccer nets sold at Wal-Mart, Ace Hardware and other national sports and toy stores from May 2002 through May 2008. The fixed knot flexible openings in the soccer goal are five inches instead of a safer four inches. Two reports of entanglement have been reported including the strangulation death of a 20 month year old child. The nets are sold under two different brand names, MacGregor Folding Soccer Goals and Mitre Folding Soccer Goals. The MacGregor nets measure 6 feet wide by 3 feet high and the Mitre net measures 8 feet wide by 6 feet high. Both nets have a foldable white frame with a white net that is attached by Velcro strips. MacGregor consumers should check their assembly instructions for model number 97236 and on the box for an UPC code of 029807972365. Mitre consumers should check their assembly instructions for model number 89186 and the box for an UPC code of 029807891864. Bridgestone is Recalling Nearly 200 Tires in Fear of Tread Detachment

184 tires are being recalled by Bridgestone because they do not comply with the Federal Motor Vehicle Safety Standard No. 119. That standard states that all tires for motor vehicles over 10,000 pounds must be branded with the maximum speed limit rate. The tires being recalled have not been branded and therefore the tread/belt can detach from the tire when used at speeds above 55 MPH. A tread detachment is a serious event that usually leads to the driver losing control of the vehicle which ultimately ends in tragedy. The tires in question are Firestone Brand FS400, size 315/80R22.5 Load Range L, Commercial Truck Tires manufactured between June 15 and June 28, 2008. Firestone dealers will replace the tires affected by the recall and remount the tires free of charge. Retailers are Recalling Simplicity Brand Cribs due to Suffocation Hazards

Simplicity Crib National retailers are voluntarily recalling 600,000 Simplicity Brand Drop Side Cribs due to a sizing problem in the crib’s hardware. The difference in size can permit the drop side to come off the track and create a hazardous gap. If a small child sleeping in the crib was to encounter the gap they could become trapped or suffocate. The cribs were sold at national retailers including but not limited to Target, Wal-Mart and Babies “R” US between January 2005 and August 2008. Affected Simplicity models include Aspen, Crib N Changer Combo, Gabrielle, Camille, Providence and Shenandoah. Simplicity crib owners should check the label on the headboard under the mattress support for model numbers 8620, 875, 8748, 8755, 8756, 8778, 8810 or 8994 and for date codes that end in 05DH, 05GB, 06DH, 06GB, 07DH, or 07GB. If your crib is affected by the recall stop using it immediately and seek different beds for your children. Recalled cribs can be returned to the place of purchase for a refund or store credit. Deere & Company Recalls 2,500 Utility Vehicles due to Gas Leak

Deere & Company has voluntarily recalled 2,500 Gator XUV utility vehicles due to concern of a fire hazard. The fuel tank can leak from a gap in the seam at the base of the filler neck posing a fire hazard. The recalled vehicle is model 620i and was sold nationwide from April 2008 through August 2008. The model number can be found painted on the side of the vehicle. Consumers should also check the serial number plate that can be found on the vehicle’s frame in front of the passenger side rear fender. The following are serial numbers affected by the recall:

General Motors Corporation Has Recalled over 850,000 Vehicles

General Motors Corporation has recalled over 850,000 vehicles due to the potential electrical failure that could lead to a vehicle fire. The vehicles in question are equipped with a heated windshield wiper fluid system. When used, the system could potentially short circuit and cause other electrical functions to malfunction greatly increasing the risk of a fire. The recall involves the 2007-2008 model year Chevrolet Silverado, Tahoe, Avalanche and Suburban, Cadillac Escalade, Escalade ESV and Escalade EXT, GMC Acadia, Sierra, Yukon, Yukon XL and Saturn Outlook; 2006-2008 Hummer H2, Cadillac DTS and Buick Lucerne; and the 2008 Buick Enclave. If you drive those vehicles and notice a strange odor or see smoke when using your windshield wipers please return the vehicle immediately to a dealer to have the system fixed free of charge. Dealers will install a wire harness with an in-line fuse system to solve the problem.
